At UCA we recognize that privacy is important. This document outlines the types of personal and protected information that we coordinate and manage on behalf of patients as they access UCA-affiliated centers.  We hope this will help you make informed decisions regarding the UCA family of centers and the role that we will play as care and records custodians.

Scope

This notice offers highlights of the full UCA Privacy Policy, which describes in detail the privacy practices that apply to all UCA centers and anyone associated with UCA through any contracted relationships for services provided at any UCA center worldwide (collectively, UCA network "services"). Personal information and other data we collect

    • UCA affiliated centers collect personal and protected information from patients through the normal process of providing services to patients.  UCA and any affiliated center staff require personal and protected information in order to provide the medical services required to perform as expected.  This information is processed through central repository that is securely protected with patient privacy as our primary concern.  Any use of this information is strictly internal to the UCA network and that use is directly linked to the continuous process improvement which results in better service to patients.
    • UCA's servers automatically record information when you visit our website or use some of our products, including the URL, IP address, browser type and language, and the date and time of your request.

Uses

We may use personal information to evaluate trends in disease management processes and evaluate the need to add or delete services.  Information is not evaluated personal to any single patient except as required by a provider to facilitate treatment or for staff to bill and collect from insurance companies or other third parties.  These uses are all covered under the authorization process that patients initiate during the treatment process.

    • We may also use personal information for auditing, research and analysis to operate and improve UCA technologies and services so that we can better serve patients.
    • We may share aggregated non-personal information with third parties outside of UCA if such information will assist in improving the care to the patients that we serve. When we use third parties to assist us in processing your personal information, we require that they comply with our Privacy Policy and any other appropriate confidentiality and security measures.
    • UCA processes personal information on servers operated by third parties and located only in the United States of America.

Choices

Patients that have chosen seek care at a UCA affiliated center have authorized personal to access information in order to perform their job as it relates to care, billing, collection, follow-up and care management.  You can find more information about your choices in the privacy notices or FAQs for specific services.

We make good faith efforts to provide you access to your personal information upon request and to let you correct such data if it is inaccurate and delete it, when reasonably possible.
